Home Delivered Meals:
This program provides homebound individuals 60 years and over a balanced meal of a protein, vegetable, fruit, bread and milk.
To qualify you must be 60 years and over, homebound, have a nutritional risk, live in the Moab Valley in Grand County and have an assessment mandated by the Utah State Aging.
To qualify for Home Delivery you must have a prescription from your physician, and an assessment from the Nutrition Program Manager.

Transportation:
The Center provides transportation for appointments. The center also provides weekly trips to City Market on Tuesdays at 12:30 pm - There is limited seating so please call (435) 259-6623 to get on the list

A suggested donation of $2.50 is accepted for anyone over 60.
